• Drivers license or valid photo identification

• Payment for any applicable co-pays

• Any relevant medical records or other studies, including lab work

We require 24 hours notice for cancelled appointments.

Co-pays are due at the time service is rendered.
As a courtesy your insurance will be billed for additional charges.
